Please complete the intake paperwork prior to attending your first appointment with your therapist to streamline the process and focus on addressing your needs. The Informed Consent and Release of Liability forms are for your therapist to contact your family doctor, psychiatrist, previous mental health provider, a support person to be involved in your treatment process, etc. Please notify your therapist if you need additional release forms for another provider, family member, etc. If you feel the document doesn't apply to you, please simply mark n/a and move on.
The entire intake must be completed in the system for your file to be complete and viewable by your therapist. In order to keep your scheduled intake appointment, your therapist requires these documents to be completed and able for them to review at least 24 hours prior to your scheduled appointment. If you are unable to complete these documents within 24 hours of your scheduled intake appointment, no worries, but be sure to communicate this with you therapist so that your appointment can be rescheduled in a timely manner.

I work with teen and adults struggling with a variety of symptoms/barriers. Some include depression, anxiety, bipolar disorders, developmental trauma, complex PTSD, LGBTQIA+ issues, self-esteem, attachment issues, stage of life issues, personality disorders, chronic stress, and disrupted wellness practices.
I implement an integrative approach in my sessions. I apply concepts and skills from Trauma-Informed Care, Mindfulness-Based Cognitive Behavioral Therapy, Systems and Attachment Theory, Compassionate Inquiry, Acceptance and Commitment Therapy, Dialectical Behavioral Therapy, Schema Therapy, Internal Family Systems, Somatic Healing methods, and Feminist theory. My therapeutic process is rooted in psychoeducation and the mind-body connection. I completed an intensive Mindfulness-Based Cognitive Therapy (MBCT) training and became a clinician trained in the Collaborative Assessment and Management of Suicidality (CAMS). My practice is Anti-Oppression Informed Practice certified.
